I acknowledge that I have voluntarily applied to the Alliance for the Arts (hereafter “the Alliance”) to participate in activities at their facility located at 10091 McGregor Blvd, Ft.Myers, FL 33919.

I am aware that there may be hazards associated with these activities and I am voluntarily participating in them with the knowledge of those potential hazards. I hereby agree to accept all risks of injury and death, and verify this statement by checking the above consent.

I agree to hold the Alliance and any of its subsidiaries or associate organizations or persons acting on its behalf harmless for any bodily injuries sustained or for loss or damage of any personal articles while on the premises or participating in any activity sponsored by the Alliance, its subsidiaries, or associate organizations on the Alliance grounds.

I hereby release and forever discharge from any claims whatsoever which arise or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the participation in activities at the Alliance today.

I hereby agree that the Alliance may use my photographic likeness taken during my participation in any Alliance activities for any purpose including for use in promotional materials and on the internet.

I recognize that any confidential information shared with me as part of my duties as a volunteer shall not be divulged to unauthorized individuals, agencies, or organizations.

I have carefully read this AGREEMENT and I fully understand its contents. I am aware that this is a release of liability and a contract between me and the Alliance. I am signing this document of my own free will.
